We are Soul Care Home Health Services, a leading Chicago-based home health care company, and want you to apply your energy and skills to this dynamic environment and become an integral part of a caring, professional Job Description Job Responsibilities Conducting initial nursing evaluation visit in the patients home Following patient for weekly visits per patient's needs and physician orders Develop, coordinate and revise the treatment plan based on physician recommendations Prepare accurate timely reports and required documentation Adhere to all practice standards and best practices as they apply to patient care Job Requirements - Ability to drive within 30-60 miles daily - Excellent charting skills (we use EMR) Home Health experience preferred - Bilingual nurses a plus (Spanish, Polish, Tagalog, Gujarati, etc.
