Social Worker
Preferred Hospice is looking for an additional Social Worker for their Hospice Program! Summary of Position Responsibility:
Essential Duties and Responsibilities include the following:
Assesses the social, financial, physical, resource and emotional needs of patients and families.
Supports and educates patients and families regarding end of life issues and advanced care planning.
Attends and participates in interdisciplinary team meetings for patient/family care planning.
Participates in patient chart audits.
Attends on-site care plan conferences for patients we serve at facilities.
Uses social work lead as a clinical resource and for assistance in caseload management.
Other duties may be assigned or required for the performance of this position.
Experience and Skills:
Education & Formal Training Required:
-Masters in Social Work preferred -Bachelors is Psychology, Social Work, or Sociology -Training in patient assessment and counseling -Training in crisis management -Maintains professional licensure requirements Call now, make a career change & make a big difference in someone's life!!
